This exam is needed by nurses, care workers and nursing assistants from non-EU countries. Without an official language certificate, it is not possible to obtain professional recognition (Berufsanerkennung) and a work permit in Germany.
The exam can also be taken by EU citizens with a qualification in elderly care (Altenpflege), since this profile — unlike general nursing — is not automatically recognised.
Formally, the exam is open to everyone — there are no restrictions based on age or citizenship. It can also be taken by administrative staff at hospitals, insurance companies, or anyone who wishes to officially prove their knowledge of medical German.
Sufficient in some federal states for Berufsanerkennung. Working with certain restrictions is possible.
Full recognition of qualifications in most federal states. In Bavaria, exactly B2 is required for Berufsanerkennung. Work as an examinierte Pflegefachkraft — with a higher salary (TVöD P7–P8 instead of P5 for assistants). Opens the path to career advancement: senior nurse, mentor, ward manager.
Available since 2019 — only B2 level. Difference from telc: the receptive parts (listening and reading) are conducted in a digital adaptive format — the tasks automatically adjust to the participant's level. Recognised by authorities, but less commonly encountered.
Without medical topics. Many nurses choose these, considering the texts easier. However, if the recognition process requires a Kenntnisprüfung, medical language is necessary anyway — and it is better to prepare for the specialist exam from the start.
Conducted by state authorities. Currently available in Berlin, Bremen, Hamburg, Mecklenburg, NRW, Rhineland-Palatinate and Schleswig-Holstein. From 2024, the FSP is the only officially permitted format in a number of federal states. Tests language only — no medical specialist knowledge. Three parts: conversation with a patient, conversation with a colleague, written task.
The exam fee is approximately 165–205 € depending on the centre.
The exam consists of two parts — a written and an oral part. Both take place on the same day.
Audio with professional topics from the world of medicine and care. Important: the audio is played only once.
Reading texts on medical topics and filling in grammatical gaps in a professional context.
Choose from: Aufnahmebericht (patient admission report) or Biographiebericht (biographical report). A medical document must be written based on the provided data.
Account of an experience or picture description, followed by questions from the examiner.
Kurzvortrag — short presentation on a medical topic with a clear introduction and conclusion, followed by questions from the examiner.
Discussion with the exam partner on a controversial topic from the field of care. Important: you should not simply agree, but express and justify your own opinion.
This is a dual-level exam — depending on your result, you receive either a B1 or a B2 certificate:
If B1 level is achieved in the oral part and in at least one section of the written part (listening, reading or writing).
If B2 level is achieved in the oral part and in at least one section of the written part.
Writing too creatively — what is required is the formal style of a medical document, not an essay.
Forgetting mandatory fields: admission date, complaints, medical history, allergies.
Literal translation from the native language → unnatural constructions in German.
The audio is played only once — many are not prepared for this and lose their footing from the first seconds.
Panic at unfamiliar terms → the thread of the entire text is lost. You need to learn to understand from context.
In Part 3 (discussion), agreeing with everything — no real dialogue, assessment drops.
Kurzvortrag without structure: no introduction and no conclusion — comes across as a stream of thoughts.
Nervousness → medical vocabulary is forgotten, switching to simple everyday language.
Spending too much time on one task — not enough left for the others. You need to move on and come back at the end.
